The following information is needed before your child can enroll at Treadway:

In County transfers:

• Proof of residence (this must be some type of document with the physical address and parent's name on it).

Out of State transfers or new entry into Kindergarten:

• 1. Birth certificate
• 2. shot record (on blue card)
• 3. physical (on yellow form and must be within a year of entrance),
• 4. proof of residence (this must be some type of document with the physical address and parent's name on it, not a driver's license), and
• 5. social security card or number. A Kindergarten child must be 5 by September 1 of current/entering school year.
• NOTE:
• Starting the school year for 2008-9 all Kindergarten entries must have 2 varicella immunizations (this is a new law going into effect for next year kindergarteners, this means pre-K students will need an additional immunization)
• Immunizations required: 5 DPT, 4 Polio, 2 MMR, 2 Varicella, 3 Hepatitis B

Once this information is obtained you can either come to the school and pick-up the appropriate paperwork to be completed or download and print the necessary forms below.

When all requested paperwork is completed and brought to the school your child may start immediately.
