Becoming an East Ridge High School Athlete

We’re so excited you’re considering becoming an elite member of one of our athletic teams. Below you will find a list of information and forms required in order to participate in any sport at East Ridge High School.

      - Lake County Schools Sports Screening Physical Examination
      - Consent and Release from Liability Certificate
      - Consent for Cognitive Testing and Release of Information
      - EKG Screening Permission Form
      - Concussion and Heat Related Illness Information Release Form
      - Emergency Treatment Authorization Card (Note: Both halves must be completed.)
      - Copy of Insurance Card
 
Please remember that all freshman or transfer students must also submit a copy of their birth certificate with their physical paperwork.
