How do I enroll a student?

  1. A parent or guardian must come to the guidance office and fill out enrollment paperwork.
  2. You should bring transcripts from your former school or information to request transcripts from the former school.
  3. You will need to show a birth certificate.
  4. You will need custody papers if applicable.
  5. You will need an immunization record withfive DTP shots, four polio shots, TD shots, two MMR shots, and two or three Hepatitis shots.
  6. If the student is coming from out of state, we need a physical that is no older then 12 months.
  7. If student is under ESE, we will need a current IEP.

How do I withdraw a Student?

  1. A parent or guardian needs to come in to the guidance office to withdraw a student. You cannot withdraw a student over the phone - we need a signature.
  2. Students need to return all books and other items (uniforms, etc) that belong to the school.
